This NOFO will award up to $350 million in FY 2026 funding via cost reimbursable grants. The actual amount available to be awarded under this notice will be subject to the availability of funds. Eligible States (See Additional Eligibility information below) with greater than 14 percent of total bridges classified in poor condition are to receive no less than $32.5 million, provided the State submits eligible application(s) with a total value that meets or exceeds $32.5 million. There is no minimum award amount for States that do not meet this condition. No State shall be awarded more than $55 million. The successful applicant will be expected to collaborate with ACL in the design and implementation of these demonstrations in the scalability of falls prevention and related chronic disease management interventions through approximately three advanced community care hubs that support care transitions and screening for the risk of falls. Grants awarded under this announcement are to provide disaster reimbursement and assistance funds to those State Units on Aging (SUAs), and federally recognized Tribal Organizations who are currently receiving a grant under Title VI of the Older Americans Act (OAA), as amended. Total funding available for disaster assistance is subject to the availability of funds appropriated. The estimated number of awards is seven with a one year project period. Funds may only be used in those areas designated in the Major Disaster Declaration issued by the President of the United States under the Robert T. Stafford Relief and emergency Assistance Act.
